HAK-Support-Community (HAK-SC) is a nonprofit organization based in Lewiston, Maine. Founded in 2025 by Patrick Nyembo, we are committed to reducing social isolation and ensuring vulnerable individuals can access vital care, resources, and community support.
We provide free transportation to medical appointments, grocery shopping, community events, and other essential destinations. We also offer safe shelter and recovery support for homeless individuals discharged from hospitals who need a compassionate place to heal.
Our mission is to help people regain independence, restore dignity, and feel connected to the community they call home.
